POSITION SUMMARY:
Responsible for accurate and timely completion of MDS for each resident to satisfy regulatory requirements. Key part of the Senior Living Center management team, responsible to assist with scheduling resident assessment as needed. Hands on knowledge of current resident care issues required. RN weekend coverage is included in this position.

ESSENTIAL JOB FUNCTIONS:

Responsible for accurate and timely completion of MDS upon admission, every 90 days, annually, and upon a significant change of resident condition.

Responsible to participate in weekly care plan meetings, make care plan edits in the computer, and print out accurate and timely changes.

Responsible to complete Rap and Triggers in a timely manner and bring same to team care plan meetings to assure comprehensive care planning.

Attend shift-shift report to enhance knowledge of day-to-day condition of each resident and communication between nurse and nurse aide team.

Responsible for establishing and maintaining effective professional relations with patients, families and caregivers.

Responsible for establishing and maintaining effective working relations with other healthcare workers, area personnel and management.

Responsible for related duties as assigned/required; participating in continuing education/ special projects and staying informed on trends/developments in the field of nursing.

Maintain strict patient confidentiality.

Demonstrate positive attitude of customer service.

Other duties as assigned or needed for the smooth operation of the Senior Living Center.

Required to work the floor as Charge Nurse every other weekend.

Required to take rotating call 7 days at a time.

EDUCATION/EXPERIENCE:
Nursing degree from an accredited program, diploma from an accredited program, current RN state license, and annual CPR certification. Medical/surgical, rehabilitative and/or long-term care experience is preferred. MDS experience preferred. Must have acceptable background check.

SKILLS:
Nursing skills in medical/surgical, knowledge of long-term care regulations and MDS completion and formulation of resident care plans and evaluation process needed. Understanding of safety precautions and infection control procedures for dealing with sharp instruments, needles, blood, and body fluids; understanding of good body mechanics to avoid injury; excellent communication skills; strong problem solving abilities; positive interpersonal skills and sensitivity; well organized; self directed; able to coordinate well with others; able to use all related medical equipment, machines, monitors and computer applications.

SUPERVISORY RESPONSIBILITIES:
May supervise LPNs, other RNs, CMAs, CNAs.

EQUIPMENT TO BE USED:
Computer, care plan and MDS, Rap and Trigger forms, SA02 machine, med and treatment carts, bed and chair scales, Sara lift, Hoyer lift, phone, intercom, fax, shredder, copy machine, whirlpool, stretcher, suction machine, hospital beds, concentrator, maxi-mist machines, crutches, splints, etc.

TYPICAL PHYSICAL/MENTAL DEMANDS:
Must speak clearly; repetitive motion using wrists, hands and/or fingers; average hearing and vision; must be able to use good body mechanics to: lift up to 20 pounds frequently and up to 100 pounds occasionally; to maneuver medication cart and various equipment (weight 150 to 175#), be able to reach, turn, and/or bend as necessary to perform care and treatments of residents, some of whom may be in bed; must be able to be on feet on concrete hallways up to an 8-hour shift, to think clearly and manage a large amount of information quickly, prioritizing and handling patient, resident, family and/or staff concerns in order of importance; must have ability to triage and manage all information effectively; reasoning ability; mathematics ability; language ability for large quantity of reading, report preparation and communication.

WORKING CONDITIONS:
Works in an environment with biological, chemical and mechanical hazards. The employer will provide appropriate training, protective equipment and engineering controls for the protection and safety of the employee.
